You may come in to the Financial Aid Office to pick up these forms or you may download them to your own printer. For 2007-08 every student must submit a Questionnaire and a Student Authorization form.

About 30% of students are selected for verification. You will be informed if you need to provide a signed verification worksheet and proof of income, usually a signed copy of your and your parents\' (if you are dependent) 2006 income tax returns. You may download the Verification Worksheet to submit with the tax returns.

Petition for Extension of Financial Aid 1 Use this appeal form if you have been disqualified because of not completing 75% of your attempted units or maintaining a 2.0 GPA. It must be submitted with an educational plan from a counselor
Petition for Extension of Financial Aid 2 Use this appeal form if you have an Associates Degree or more than 90 attempted units. It must be submitted with an educational plan from a counselor.
